Granting Access to Your Information in Case of Incapacitation

Call us to add an authorized representative or a POA to your Profile. A Via Benefits representative asks a few questions, reads a statement, and collects your verbal agreement authorizing the appropriate type of representative for your needs. There’s no form to fill out unless you can’t come to the phone.

If you're incapacitated or unable to come to the phone and verbally authorize your representative, we can email you or your legal representative an Authorization to Release Protected Information Form to fill out. 

Your legal representative can complete and return the form along with the legal documentation of representation (for example, Durable POA, Guardianship, or Conservatorship). The legal documents need to be sent to us for review before the POA can provide assistance. 

You can send legal documents to us by email or mail. Please clearly provide the participant's name and date of birth on the documents. We're unable to associate the document with the participant's Profile if we can’t identify the participant on the document.
