Each health plan has a Summary of Benefits and Coverage (SBC) that outlines the services and benefits covered by the plan, any limitations or exclusions, and the associated costs. Reviewing the SBC can help you understand your coverage and compare plan options.
Before You Enroll
Our licensed benefit advisors are familiar with SBCs and can help you compare plans and choose one that fits your needs and budget. For your convenience, you can also review a plan's SBC before enrolling.
Individual and Family Plans
To find an SBC for an Individual and Family plan you're considering, go to marketplace.viabenefits.com. After answering a few questions, you’ll be presented with plans in your area. Select the option to view more details next to the plan you want to review. Then, in the Overview section under Plan documents, select Summary of benefits. Medicare Plans
To find an SBC for a Medicare plan you're considering, go to my.viabenefits.com/shop. After answering a few questions, you’ll be presented with plans in your area. Select View Plan Details for the plan you want to review, then select Plan Documents. Under Plan Documents, select Summary of Benefits. After You Enroll
After you enroll, contact your insurance carrier directly if you have questions about your coverage or want to confirm whether a specific service or procedure is covered. The carrier’s contact information is usually listed on the back of your insurance ID card.
If you need another copy of your SBC, you can:

Log in to your carrier’s website. Many carriers provide a digital version of the SBC.
Contact Via Benefits Insurance Services for assistance. A representative can assist you in calling your insurance carrier.
