When your enrollment is complete and your health plan is issued, your new insurance carrier mails you a Summary of Benefits document. The document provides a snapshot of what's covered by your health plan. If you have specific questions regarding the coverage provided by your health plan or want to know if a specific procedure or service is covered, contact your insurance carrier directly. Your insurance carrier's contact information is generally on the back of your insurance ID card.
If, for any reason, you need to replace your Summary of Benefits document, you have the following options:
Call your insurance carrier directly. The insurance carrier's phone number is generally on the back of your Insurance ID card.
Log in to the carrier’s website to view expenses or see a digital version of the Summary of Benefits.
