Your Medicare Card
Your Medicare card shows you have Medicare health insurance. It includes the dates you became eligible for Part A (Hospital Insurance) and Part B (Medical Insurance). Go to medicare.gov to get started with Medicare. Medicare cards are used when enrolling for Medicare insurance plans, seeking medical care, or inquiring about a Medicare claim. Presenting a Medicare card when seeking medical care ensures a claim for payment is sent directly to Medicare.
Medicare card tips:
Take your card with you when you travel, in case you need emergency medical attention.
Always use your exact name and Medicare Number when inquiring about a Medicare claim. Married Medicare beneficiaries each have their own cards and Medicare Numbers. Don't use your spouse's information.
Keep your Medicare Number protected, as you would a credit card or bank account number. Never let anyone else use your Medicare card for any reason.
If your Medicare card has incorrect or out-of-date information, or has been lost, stolen or damaged, you can request a replacement card by going online or calling Medicare.
